Tracfone has an extra layer of security inbuilt in firmware. Such phones can't be unlocked by simply entering an unlock code and have to be reflashed.

A few hardware tools support tracfone unlocking.
Infinity box supports motorola and some LG locked to Tracfone
MSS box supports motorola locked to Tracfone
